A flu vaccination clinic will be held on our campus on October 7th. Flu vaccination consent forms will be sent home with students by end of day on Wednesday. Parents must compete the form in its entirety and return the completed form to the clinic. Copies can also be obtained in the front office, or can be downloaded below:
